New Questionnaires - Provide guidance and/or samples

When you guys add items, you never provide context or direction to a tutorial. Your system just flags things as not completed, yet we never knew about it. This needs to be changed somehow to notify users of new questions or information you will be asking so we can be better prepared with the information. Also, when things are flagged, you don't specify WHAT the issue is, you just expect us to magically know.
